THE TEAM EXCHANGE is comprised of Geriatric Psychiatry Programs throughout Ontario, Canada providing specialized assessment, consultation, treatment and education to older adults, their families and service providers.
Programs are also mandated to provide community development and advocacy for the target population. The majority of services utilize a community focused outreach model. Programs are interdisciplinary and may comprise a combination of Social Work, Nursing, Occupational Therapy, programs may have the use of a Consulting Psychiatrist and/or Geriatrician.
If you are aware of an individual who is experiencing difficulties with an age related illness in combination with a mental health problem or if you require further information one of the programs listed below may be able to help.
